MEDNET©
Created in 2000, the MEDNET© program is a community-based initiative to increase access to prescription drug services for uninsured and medically needy residents in Pinellas and Polk County. MEDNET© utilizes clinic-based Patient Advocates to assess patient medication needs; identify compassionate use programs; complete and submit program applications; and receive requested medications for program clients. Suncoast Health Council, Inc. utilizes proprietary MEDNET© software to coordinate program services and quantify the annual retail value of medications received.
MEDNET© has been recognized by Allegany Franciscan Ministries as a best-practice model for pharmacy access in the Tampa Bay Region. The MEDNET© Program has been presented at several national conferences and was recently presented at the Third International Living Knowledge Conference in Paris, France. Breast Health Navigator Project
The Breast Health Navigator Project provides care coordination services to patients diagnosed with breast cancer to insure that they understand treatment needs/options and how, when and where to secure treatment services. The Breast Health Navigator works from an office located at Bayfront Family Health Center to offer individual counseling, support, and treatment referral to patients diagnosed with breast cancer, including referral to ancillary support services. An example of a referral to ancillary support services will include referrals to treatment support programs that provide financial assistance for the payment of health insurance premiums, prescription drug fees, transportation, childcare, or other services needed to promote and sustain access to breast cancer treatment. Eligible patients include current patients accessing care at Bayfront Family Health Center, as well as non-Center patients referred for breast cancer treatment by the Mammography Voucher Program or any other community- or clinic-based referral source. Care coordinated patients are eligible to access services for up to one year. Patients may be insured or uninsured. The grant provides five (5) uninsured patients with financial support to pay up to $1,000 (per patient) to cover a surgeon’s fee to insert a central line to administer chemotherapy drugs.

Insurance Services Program (ISP)
An enrollment-based program designed to maintain private health insurance coverage for People Living with HIV/AIDS. The Health Councils, Inc. created the Insurance Services Program (ISP) in February 2002 to accompany the AIDS Insurance Continuation Program (AICP). Both programs provide for the payment of health insurance services (e.g., premiums, prescription drug co-payments, etc.) for People Living with HIV/AIDS in an eight county area. However, there are differences in eligibility requirements and service benefits between the two programs. In general, the AICP provides greater support for the payment of health insurance premiums, while the ISP provides greater support for the payment of prescription drug co-payment fees. Collectively, the two programs provide benefits in excess of $1.4 million annually, funded in part by General Revenue funds provided by the State of Florida and federal funds that flow to the State of Florida through the Ryan White Care Act.
Clients of the ISP program differ dramatically from the clients of other Ryan White-funded programs. Specifically, ISP program clients are not People Living with HIV/AIDS (PLWAs), but rather the case managers who coordinate the delivery of HIV/AIDS program services for PLWAs in the eight county service area. ISP program clients are employed by community based AIDS service organizations that provide case management support at the local level or by the various public health offices located throughout the ISP program area.

Treatment Support Project
The Treatment Support Project provides financial support to patients diagnosed with breast cancer to insure that they can actively participate in their treatment plans. The support is in the form of a patient stipend limited to $500 per patient per year. Eligible patients qualify for vendor payments to cover expenses related to accessing health care, including deductibles, co-pays, COBRA/insurance premiums, medical supplies, home health and treatment support services (child care). Patients are required to provide documentation of financial need and expenses to be eligible for the stipend. Case managers and/or health care providers should complete the application and verify eligibility, and arrange for provider’s acceptance of vouchers. The Health Council of West Central Florida (HCWCF) acts as the fiscal agent processing applications and issuing payments directly to the vendor.
